Clackmannanshire Reablement Service

Clackmannanshire Council Reablement Service is a care at home service with a focus on recovery and regaining daily living skills. Social Services can arrange and provide, in some circumstances, a reablement care service to enable people to regain or maintain their independence in their own home. People are normally referred to the service following a hospital admission to assist on discharge or following a change in their health or wellbeing at home.

Older Adults

The service works with people to assess their needs over a short period of time. A reablement carer can assist with a wide variety of daily living tasks, including personal care and tasks such as dressing, washing, getting to the toilet, preparing meals and supervising medication. As the reablement service is a short term only service, if ongoing care is required at the end of the reablement period the service will arrange this with a home care provider.
